In support of quality improvement initiatives at your site, IIHO can conduct a deep dive analysis for your team. This might involve a patient-level data extract for your site on specific indicators or could involve a deeper investigation into site, zone or provincial trends through aggregate analysis.

Please contact IIHO and include the following details to guide the deep dive data request, or fill out the Deep Dive Request Form and attach to your message:

• Time frame (date from and to)
• Timeframe reference, if trending (e.g. fiscal year, calendar year, fiscal quarters, calendar quarters)
• Site and/or zone and/or provincial
• Cohort of interest – Primary and/or revision (together or separate), Hips and/or Knees (together or separate)
• Any other patient characteristics (e.g. PHN, admit date, surgery date, discharge date, procedure type, side, medical events, mechanical events, deep infections, transfused yes or no, readmission within 30 days discharge, ER visits within 30 days discharge)
• Date by which you need the request completed

For adding a new measure, please indicate:

• Name of the measure and short description
• Quality dimension (Acceptability, Accessibility, Appropriateness, Effectiveness, Efficiency, Safety)
• Weighting
• Performance range
• Date that data collection will begin  *Note: the weighting of all measures on the scorecard should equal 100. If you remove and/or add measures, you may need to adjust the weighting of the remaining measures. Please consider these adjustments and indicate the changes.
