Document Change Request

Process for Submitting a Document Change Request

All provincial forms, guidelines and care paths developed by the H&K OA surgical program are reviewed and updated annually, based on latest and best evidence. The Clinical Committee considers all change requests and supporting evidence, prior to recommending any amendments to the provincially standardized practices.

All program participants and their colleagues, including frontline providers, have the right to put forward a request for a change to the provincial documents. When submitting a change request, please follow the steps outlined below:

  1. Gather as much scientific or data evidence together as you can to support the reason for your question, request or recommendation.
  2. Summarize the evidence and your ‘ask’.
  3. Submit these items to IIHO by June 30 of each year.
  4. IIHO will review them with the chair of the Hip and Knee Replacement Clinical Committee.
  5. If the topic is approved for clinical committee discussion, IIHO will arrange for more in-depth investigation of evidence, as required.
  6. The Hip and Knee Replacement Clinical Committee meets annually and their agenda will vary annually depending on the relevant discussion points of the time.
  7. Decisions made at the Hip and Knee Replacement Clinical Committee meeting are documented by IIHO.
  8. The corresponding changes to the care path are made by IIHO.
  9. The revised care path, with the changes highlighted, is circulated to those on the mailing lists.

Finally, team representatives communicate these decisions to all of their local colleagues and support implementation of the changes in their local work flow.
